Our Lady of the Mountains offers several options to memorialize family or friends, or to commemorate a special occasion or event. Engraved bricks and pavers and engraved benches provide support for the preservation of our historic Church, serve as a lasting recognition or memorial, and become part of the enduring history of Our Lady of the Mountains Catholic Church.

Important Note! If you close this page without submitting your form, you will lose the information you entered.
Your memorial will be ordered after payment has been confirmed. You may drop your payment by the Parish Office or pay through VANCO online by clicking or scanning the QR code.
When paying online, navigate to the Payments block and select the down arrow in the "Choose One" dropdown to see payment options for engraved Memorials.
Bricks come in two sizes; 4x8 inches and 8x8 inches. A 4x8 brick provides 3 lines and an 8x8 brick provides 6 lines of up to 20 characters each. Spaces and punctuation are included in the character count.
Plaques are 12x12 inches and provide 3 lines of 16 characters each, including spaces and punctuation.
You may order up to 3 4x4 bricks, 2 8x8 bricks, and 2 pavers on any one form. If you need to order more in any one category, you will need to submit another form. If you need to submit multiple orders, please make sure you provide the same Full_Name, Email, and Phone on each.
The form fields should be self-explanatory. Brick and Paver placement is constrained by the mix of differing sized memorials in a given space. If you have a preference on where your Brick or Paver appears, please note it in the Additional Information text box on the form. We will strive to place your Memorial where requested, but we cannot guarantee it due to the aforementioned constraints.
