Faith Formation Registration

Please fill out all the information you can on this Registration Form and we will place your child in the appropriate class.

Enter the first and last name of the parent or guardian registering the children. This field as well as the next two fields (Email and Phone Number) are required to identify the children's records. In the same manner you may (but are not required to) enter the second parent and an emergency contact.

If you only need to change a child's information or register additional children, you need only to fill out the Parent or Guardian name, phone number, and email and then skip to the child registration section.
